This clinic is designed to empower patients with the knowledge to make informed decisions regarding their disease to prevent diabetes related complications and improve quality of life. The Diabetic Champions Program has been designed using ADA/AACE/Endocrine society guidelines for patients who have been diagnosed with Pre-diabetes or Diabetes. Patients will undergo screening tests for comorbidities related to diabetes mellitus. Patients will be given Diabetes education pertaining to diet, exercise, and complications. Patient will gain advanced knowledge of glycemic management, able to recognize complications of Diabetes and understand when the health care team needs to be contacted.
GOAL:The main focus of this program is to achieve target HbA1C of <7%, reach goals of preventative and quality indicators as well as early detection of complications to further delay or prevent diabetic related co-morbidities and hospitalizations.

This clinic has created a Chronic Disease Management Program for COPD patients to improve and sustain quality of life. The Breathe Well Program is designed to provide patient education for effective COPD self-management. Patients will undergo a unique COPD management plan which is specific to each patient, helping minimize emergency room visits and hospitalizations. Patients will be given COPD training pertaining to how to use inhalers and nebulizers. Patients will gain knowledge emphasizing COPD education including medication adherence, prevention and treatment of exacerbations.

This clinic has established a Congestive Heart Failure Program to ensure patients receive optimal care and understand their treatment options. Patients will gain knowledge emphasizing Congestive Heart Failure education including; medication adherence, risk prevention and recognition of possible symptoms that develop quickly or worsen rapidly, that require the patient to seek emergency treatment. Patients will be educated about Congestive Heart Failure and directed to keep up with routine physical exams and diagnostic tests. Patients will also undergo screening tests for comorbidities related to Congestive Heart Failure such as chest pain, stress, poor circulation, blood pressure and weight.
